What is a genioplasty?

First of all, genioplasty is a cosmetic surgery operation designed to correct and reshape the appearance and shape of the chin in order to achieve a more harmonious face.

Patients generally undergo genioplasty if they suffer from a chin that is too far back, too far forward or crooked, or if they feel that their chin is too large and therefore not aesthetically pleasing.

There are 2 types of genioplasty:

Genioplasty to correct a curved chin

If the chin is a little too far forward, a genioplasty is required to move the chin back and improve the appearance of the face. There are two possible options: either the surgeon planes the jaw by filing the lower edge of the mandibular bone.

Or the prominence is cut away, in which case the front of the chin is reattached using metal plates and screws. This is called an osteotomy.

Genioplasty for a receding chin

If the chin is too far back, it is necessary to have an advancement genioplasty. In other words, the surgeon advances the chin using one of three methods:

• An advancement osteotomy, which involves cutting the jawbone to move it forward horizontally;
• A silicone graft to fill the gap;
• A bone graft, in which a bone from the body is extracted either from the pelvis, nose or even skull and then grafted to the chin. This is known as autografting.

The choice depends on the patient’s preferences and the surgeon’s indications.

Post-operative follow-up

Generally speaking, the scars left by genioplasty are almost invisible. The final result appears about 1 month after the operation, which corresponds to the disappearance of oedema in the cheeks and lips.

Finally, pain is generally moderate, and in all cases patients are prescribed analgesics and painkillers to minimise their suffering.

What precautions should be taken?

After the operation, patients are advised to :

• Eat soft foods and avoid solid foods;
• Maintain excellent oral hygiene, rinsing their mouth after every meal to avoid infections;
• Avoid food that is too hot, too acidic or too spicy;
• Avoid physical activity for 6 weeks after the operation;
• Take at least two weeks off work.

The main risks are :

• Displacement of the implant or asymmetry, which may in rare cases lead to a second cosmetic operation;
• Slight bleeding;

• An infection requiring antibiotics;
• Dental lesions, i.e. the roots of the teeth may be affected and require treatment, but this is rare;
• A risk of rejection of the prosthesis, if there is one.
