What is a genioplasty?
First of all, genioplasty means a cosmetic surgery procedure that is designed to correct and reshape the appearance and shape of the chin in order to obtain a more harmonious face.
Patients generally seek a genioplasty if they have a chin that is too receding, too prominent, asymmetrical, or if they feel their chin is too large and therefore unattractive.
There are 2 types of genioplasty:
Genioplasty for a prominent chin
When the chin is a little too prominent, a genioplasty is required to set the chin back and improve the appearance of the face. Two options are possible: either the surgeon shaves the jaw by filing the lower edge of the mandibular bone.
Or the prominence is cut away, in which case the front part of the chin is reattached using small plates and metal screws. This is known as an osteotomy.
Genioplasty for a receding chin
When the chin is too receding, an advancement genioplasty is necessary. This means the surgeon brings the chin forward using one of three methods:
- An advancement osteotomy, which involves cutting the jaw bone to move it forward horizontally;
- Placing a silicone implant to fill the gap;
- Performing a bone graft, meaning a bone is taken from the body, either from the hip, the nose or even the skull, and then grafted onto the chin. This is called an autograft.
The choice depends on the patient’s preferences and the surgeon’s recommendations.

Anyone over 18 with a chin that is too receding or too prominent can have a genioplasty.
How is genioplasty surgery performed in Tunisia?
Like any cosmetic surgery procedure, a genioplasty takes place in 3 distinct stages:
The preoperative phase
This first consultation is the ideal opportunity to ask certain questions, and to raise your doubts, fears and expectations.
It is the time to get answers to the questions raised by the procedure, before entering the operating theatre. Above all, it is an exchange aimed at choosing the technique best suited to the patient’s anatomy and expectations.
The operation
First, a genioplasty in Tunisia generally lasts between 1 hour and 2 hours, depending on the complexity of the surgery. It is most often performed under general anaesthesia, but can also be done under local anaesthesia.
If an implant is used, it is inserted either through the mouth via a small incision of a few centimetres. Another option is an incision in the fold under the chin, which is less visible than an incision on the face.
Genioplasty can also be combined with other cosmetic procedures, such as a rhinoplasty for example, or even a profiloplasty for a more harmonious face.
Finally, it requires one night in hospital, unless the medical team considers that the patient can leave immediately, in which case it is performed as day surgery.

Postoperative follow-up
The scars are placed inside the mouth or in the fold under the chin, which makes them barely visible. Swelling of the cheeks and lips takes several weeks to disappear, and the result can only be assessed afterwards.
Finally, pain is generally moderate and in all cases patients are prescribed painkillers and analgesics to keep discomfort to a minimum.
What precautions should be taken?
After the operation, patients are advised to:
- Eat soft foods and avoid solid foods;
- Maintain excellent oral hygiene with a mouthwash after every meal, to prevent infection;
- Avoid food that is too hot, too acidic or too spicy;
- Do no physical activity for 6 weeks after the operation;
- Take at least two weeks off work.
Of course, plastic surgery is never without risks. In the case of a genioplasty, the complications described are mainly infection, implant problems, bone resorption and persistent neurosensory deficit[1].
The main risks are:
- Displacement of the implant or asymmetry, which in rare cases may lead to a second cosmetic procedure;
- Mild bleeding;
- Loss of sensation in the lower lip, gum or teeth. The disturbance gradually fades;
- An infection requiring antibiotics;
- Dental damage, meaning the roots of the teeth may be affected and require treatment, although this remains rare;
- A risk of implant rejection, if an implant is used.
