What is ultrasonic rhinoplasty?
Ultrasound technology has long been used for aesthetic procedures to reduce fat and firm the skin. Rhinoplasty can now be performed using a piezotome® and ultrasonic waves, which allows nasal bones to be reshaped with great precision.
Rhinoplasty is the sixth most common cosmetic surgery procedure in the world, with 1,015,685 interventions in 2025[2], and a common procedure in cosmetic surgery in Turkey. This procedure is suitable for men and women.
The goal of ultrasonic rhinoplasty is to achieve facial harmony by correcting:
- Nose size
- Nose shape
- Nose angle
The tools used during ultrasonic rhinoplasty are powered by ultrasonic waves that vibrate the tip of the instrument to shave off excess bone.
In traditional rhinoplasty techniques, bones are scraped and chiselled aggressively. With ultrasound, randomised trials show less swelling and bruising in the days following surgery, with no significant difference in osteotomy time[3].
Before and after photos
The method
During ultrasonic rhinoplasty performed via an open approach, an incision is made in the columella, directly under the tip of the nose, to ensure better exposure and visualisation of the treatment area.
The nose is then retracted backwards by dissecting the nasal cartilage, while taking care not to cut the midline crest that separates the nostrils. The skin is then lifted to maximise exposure of the nose. The surgeon then reshapes the nasal bones using specialised ultrasonic tools; the tip and nostrils, made of cartilage, are worked with standard instruments[4].
Different tools can be used to scrape, shape and smooth the nasal bone. The ultrasonic tool works the bone with precision[4] and damages the nasal mucosa less than osteotomes[3]. Once the procedure is complete, the skin is redraped over the bone structure. The incision is closed with sutures, and a cast is applied to the nose.
What to expect?
First described in 2007 by an Italian team[5], then developed by Dr Gerbault from Paris[6], this technique involves reshaping the nasal bones using specialised tools to achieve a more refined nose shape. Here is a quick guide on what to expect before, during and after this procedure.
Before treatment
- Do not take blood-thinning medications, such as aspirin, for the period specified by the surgeon;
- Do not smoke before and after the procedure, as advised by the surgeon;
- Do not expose yourself to the sun in the days leading up to surgery;
- Do not eat or drink 6 hours before surgery[1].
During the operation
- General or local anaesthesia with sedation
- Incision made under the tip of the nose
- The nose is retracted backwards
- The skin is lifted for maximum exposure
- The nasal bridge is reshaped and refined using specialised tools that vibrate with ultrasonic waves
- Sutures to close the incisions
- An external splint or cast is placed on the nose
Immediately after treatment
- Swelling around the nose, cheeks and eyes
- Bruising and puffiness on the face.
1 to 2 weeks after treatment

Important notes
As ultrasonic rhinoplasty requires wide exposure of the nasal bones, via open or extended closed approach[4], facial swelling and bruising are expected, less marked than with conventional instruments[3]. An external splint is applied after surgery to support the underlying nasal structure, then removed between the fifth and eighth day[1].
Initial swelling subsides within a few weeks[7]. The nose can take up to 1 year to reach its final shape[7]. Make-up can be worn as soon as the cast is removed.

Ultrasonic rhinoplasty is not offered before growth is complete, around age 16[1], except in cases of injury to correct. It may also not be suitable for patients with significant bone deformities.
Risks, complications and side effects
There can be rare risks during ultrasonic rhinoplasty:
- Temporary loss of sense of smell;
- Blood accumulation under the skin;
- Tissue necrosis;
- Perforation of the nasal septum;
- Nasal dryness;
- Airway problems.

In Islam, the permissibility of rhinoplasty is debated: it is most often deemed halal when it corrects a defect or discomfort, and debated, even deemed haram, when it aims only to change appearance. The answer falls to a religious authority.

Side effects of ultrasonic rhinoplasty can include: numbness, nosebleeds, scarring, swelling, bursting of small blood vessels on the skin surface, permanent nerve damage, insufficient or excessive correction requiring revision rhinoplasty.
